Is the GUIDE Model An Option To Pay for Home Care?

One of the most common questions we hear from prospective clients is, “Does insurance pay for your services?”

Some people lack the financial resources to pay for private duty home care. But now, the Center for Medicare and Medicaid Services (CMS) has launched an initiative to help change that, at least for family caregivers of people with dementia. It’s called The GUIDE Model.

What Is the GUIDE Model?

The GUIDE (Guiding an Improved Dementia Experience) Model is a nationwide initiative launched in July of 2024. It’s really sort of an eight-year test as it will run through June of 2032.

The model is specifically designed for individuals who are the primary caregivers for someone with dementia. GUIDE seeks to improve their quality of life, reduce their strain, and help individuals with dementia remain safely at home.

What You Get with GUIDE

Imagine you are the primary caregiver for someone with dementia. For example, suppose you care for your father-in-law who lives with you and your spouse. If eligible, you will receive 19 four-hour shifts (respite visits) from July 1 to June 30. (It might be helpful to think of that as a “plan year” using insurance terminology, even though this isn’t insurance.)

That means, at no cost to you, you could schedule a caregiver from an agency like Ovation Home Care to come to your home and care for your father-in-law while you go to your own doctor’s appointment or to run errands 19 times during the 12 month period.

In fact, you can stack the four-hour shifts together. For example, you and your spouse want to attend a wedding in a neighboring town and it’s going to be an all-day affair. You could have a caregiver come from eight in the morning to eight at night. That would simply be considered three of your 19 four-hour shifts.

How the GUIDE Model Works

In the Gainesville FL area, Alivia Supportive Care is one of a select few providers approved by CMS to provide the required services, including Care Navigators to help families access the support available through this program. (Ovation Home Care has an agreement with Alivia Supportive Care to provide the respite visits delivered via the GUIDE model.)

Once you reach out to Alivia Supportive Care, they will send someone to perform an extensive assessment to determine if your loved one would be appropriate for the program. If approved, you’ll receive:

  • Care coordination and management, delivered by an interdisciplinary team;
  • Caregiver training and support, including 24/7 access to a support line;
  • Respite services, which are vital in allowing caregivers to take needed breaks;
  • And more

All told, it takes about six weeks to make sure your loved one is appropriate. Ovation Home Care provides the “Respite Services” listed above. When Alivia Supportive Care sends us the referral, we call you to set up a consultation/assessment. 

Participate families sign a service agreement with Alivia Supportive Care listed as the primary payer source. (The program provides these respite services free of charge to you, but if you choose to use our services beyond the 19 four-shifts, you’ll be listed as the secondary payer and responsible for the cost of additional shifts.)

What are the Eligibility Requirements?

Your loved one must be enrolled in original Medicare Parts A and B, with Medicare as their primary payer. Lest there be any confusion, this program is NOT available for someone enrolled in a Medicare Advantage plan or PACE (Program of All-Inclusive Care for the Elderly). 

Your loved one must also have a diagnosis of dementia, attested by a clinician on the GUIDE Participant’s roster or via a written diagnosis from another Medicare-enrolled provider. If deemed appropriate, you’ll need to “align” with Alivia Supportive Care, giving them your consent for your loved one to be a part of the program.

(NOTE: Your loved one may not be under Hospice Care to be eligible.)
